It is with great sadness that I write to inform you that I am going to be closing my practice at Imperial Medical Clinic towards the end of June 2022.

I have truly loved and cherished being allowed into your lives and I am very grateful for you entrusting me with your health. This has not been an easy decision but at this time I need to focus my attention onto my young family.

I understand how difficult the situation of finding a family doctor is and I truly apologize for any distress this news causes you. Unfortunately none of the other Doctors at IMC are able to continue your care in my absence and I recommend

• Visiting the Pathways Medical Directory (pathwaysmedicalcare.ca)
• Contacting HealthLink BC using 8-1-1 toll-free number (or 7-1-1 if you are hearing impaired) or by visiting their website at healthlinkbc.ca.
• Contacting your local Divisions of Family Practice (divisonsbc.ca), as some are establishing physician directories.

  For Burnaby patients: https://divisionsbc.ca/burnaby/for-patients/looking-for-a-doctor-in-burnaby

• Visiting a local walk-in clinic or Urgent Primary Care Centre (UPCC). UPCCs also serve as primary care clinics for people who do not have a regular primary care provider and will work to connect them to a primary care clinic as space is available.
• Asking friends and family for recommendations.

Logistically, after the 25th July 2022 I will no longer have access to reviewing any test results or letters from specialists etc. Imperial Medical Clinic will be unable to hold any responsibility for these so please do inform any specialists that you see to no longer address letters to myself/Imperial Medical Clinic or to copy me into any lab work.

Since your medical records are confidential, a copy can only be transferred to another doctor or released to you with your written permission through an authorization for release of medical record form. If you would like to receive a copy of your medical records or transfer them to another physician, please contact Imperial Medical Clinic. Please note that there is a fee associated with the transfer of medical records in the amount of $65.00. You may email completed forms to drsnrecords@imperialmedicalclinic.ca

I am thankful to you, my patients, for trusting me with your care for so many years. I have truly appreciated the opportunity to support you and your family.
